I originally bought medium it was too tight. Puppy is 6 months old and 28lbs. It is supposed to slightly lose in tummy area so it does not rub against stitches. Not loose enough direct access. You will need to unstrap everything they are out to the bathroom but I don't find it to be too bad actually
